Zusammenfassung

In einer retrospektiven Studie wurde geprüft, ob der Einsatz der dynamischen MRT der Mamma bei mammographisch eindeutigem Karzinombefund eine Mehrinformation erbringt oder lediglich einen kostentreibenden Mehraufwand darstellt. Bei 76 Patienten mit 81 histologisch gesicherten Karzinomen wurde 1 Karzinom im Stadium pT₁ in der MR-Mammographie übersehen. Gleichzeitig wurden aber 17 zusätzliche pathologische Befunde (20,9 %) erhoben, die in der konventionellen Diagnostik nicht erkennbar waren. Bei 8 Patienten fanden sich ipsilatcral und bei 5 Patienten kontralatoral zusätzliche Tumoren, die sich nach der histologischen Untersuchung als maligne erwiesen. Bei 4 weiteren Patienten zeigte die MRT kontralateral suspekte Herdbefunde, die histologisch als benigne eingestuft wurden. Das therapeutische Vorgehen wurde infolge der MRT-Befunde in 15 Fällen (18,5 %) modifiziert: Ipsilaterale Mastektomie statt Tumorektomie (n = 5) bzw. primäre Zytostase statt Operation (n = 1); kontralateral zusätzliche Tumorektomie (n = 3) bzw. Mastektomie (n = 2) bei Karzinom sowie Probeexzision (n = 4) bei gutartigem Befund.

Summary

In a retrospective study the value of MR mammography in cases of radiological well-known carcinoma of the breast was evaluated preoperatively in 76 cases of histopathologically verified carcinoma one lesion was missed by MR imaging. Compared to the results of conventional X-ray mammography additional malignant lesions were detected in 5 patients ipsilaterally and in another 8 patients contralateraly by MR mammography only. In additional 4 cases MR mammography demonstrated suspicious lesions contralaterally, but histopathology revealed benign lesions. The therapeutic procedure was changed in 15 cases (18,5 %) due to the findings of MR imaging: Ipsilateral mastectomy instead of tumourectomy (n = 5) or primary chemotherapy instead of operation (n = 1); contralateral additional tumourectomy (n = 3) or mastectomy (n = 2) by carcinoma and open biopsy (n = 4) by benign finding.
